Job Summary
Nurse Case Manager responsible for conducting and documenting initial assessments, reassessments, and significant condition changes in the EMR, developing and updating individualized care plans based on client and family goals, identifying clinical risks, initiating timely interventions, and escalating to appropriate providers. Includes field-based clinical oversight, educating and coaching Care Professionals on care needs, safety, infection prevention, documentation expectations, and red flags; communicating plan updates to Care Coordinators and staff; identifying caregiver training needs and coordinating education; providing clinical feedback on performance and adherence to plans; coordinating practitioner orders and preparing reports for external stakeholders; serving as a clinical liaison to strengthen relationships with clients, families, referral partners, and community providers; maintaining HIPAA-compliant documentation; participating in on-call coverage as required; and performing other duties aligned with licensure and business needs.
Required Qualifications
- RN license in good standing
- Current state licensure or eligibility to practice as a registered nurse
- Clinical assessment and care planning experience
- Documentation in EMR systems
- HIPAA/privacy compliance knowledge
- Field-based clinical oversight experience
- Care coordination and collaboration with teams
- On-call rotation participation (as assigned)
