Nurse Care Manager

Develop wellness and prevention initiatives and facilitate health education groups for chronic disease management. Plan, organize, and direct health education programs while collaborating with the health home team to integrate behavioral health into primary care. Participate in treatment plan development for all enrollees, monitor patient progress toward measurable goals, and address barriers to improved self-management skills. Ensure educational strategies adhere to current evidence-based guidelines and organizational mission values. Requires active registered nurse licensure in Missouri and a Bachelor of Science in Nursing.
