Job Summary
Collaborate with treatment team members to develop and implement healthcare goals for individuals with co-occurring chronic diseases. Facilitate health education groups utilizing advanced knowledge of complex medical conditions. Manage hospital and medical provider admissions and discharges for Healthcare Home clients. Train patients on medical diagnosis, treatment, and medications. Monitor and report performance measures using sophisticated information systems. Participate in initial treatment plan development for all enrollees and develop wellness and prevention initiatives. Access emergency services to provide rapid aid in case of emotional crisis. Ensure compliance with Quality Assurance procedures and cooperate with utilization review processes.
Required Qualifications
- Current licensure as a Licensed Practical Nurse (LPN), or Registered Nurse (RN) in the state of practice
- Graduation from an accredited practical nurse program
- Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check
- Completion of New Hire Orientation at the beginning of employment
- All training requirements including Relias at the beginning of employment and annually thereafter
- Current driver's license
- Acceptable driving record
- Current auto insurance
- Knowledgeable in chronic disease processes
- Excellent communication skills
- Commitment to innovative healthcare solutions
- Expertise to foster wellness and prevention initiatives
- Ability to work closely with both internal and external treatment team members
- Ability to develop and implement effective treatment plans
- Ability to facilitate health education groups
- Ability to monitor client progress through sophisticated information systems
- Ability to access emergency services to provide rapid aid in case of emotional crisis
- Ability to ensure compliance with Quality Assurance procedures
- Ability to cooperate with utilization review processes
- Ability to sit and stand for extended periods (2-4 hours)
- Ability to walk for short periods when conducting field-based activities
- Ability to frequently use hands and fingers to write, type, operate a mouse, file documents, and otherwise handle documents
- Ability to lift and/or move up to 50 pounds
