Nurse Care Manager-Primary Care (RN)

Job Summary
Coordinate with primary care providers and care team members to manage patient populations with chronic illnesses and multiple co-morbidities. Design and implement evidence-based care interventions aligned with patient preferences and functional goals while providing leadership for complex disease states. Educate patients and families on diagnosis, treatment plans, and discharge needs to support self-management and mitigate potential crisis situations. Maintain complete documentation of medical history, update care plans collaboratively, and advocate for patients across all settings. Requires Idaho RN licensure, BSN, and previous experience caring for chronic disease patients.
Required Qualifications
- State of Idaho Registered Nurse or licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the 'multistate privilege to practice' compact with Idaho
- BSN or higher nursing degree required
- Previous experience in caring for chronic disease patients required
- Regular and predictable attendance is an essential job function
- Competent to meet age specific needs of the unit assigned
- Must be able to maintain a sitting position
- Ability to lift up to 35 pounds
- Must be able to stoop, crouch or bend
Desired Qualifications
- Certification in relevant nursing area preferred
- 3 years' experience in clinical or community health settings preferred
- Previous Care Coordination, Case Management or Home Health experience preferred
- Knowledge of quality and financial outcomes of patient care processes preferred
- Previous experience with health IT systems and data reports preferred
- Previous experience with mobilizing community resources, navigating patients through the healthcare continuum, and working with disparate populations preferred
