Nurse Care Manager

Conduct comprehensive assessments, develop individualized care plans, and provide education on diagnosis, treatment, and symptom management to patients and families. Serve as a consistent clinical liaison between patients, families, and the oncology care team while coordinating care across disciplines, scheduling appointments, and facilitating timely follow-up. Identify barriers to care including financial, transportation, or cultural challenges, and deliver proactive outreach, regular check-ins, and telephone triage to ensure continuity. Document care coordination activities and patient interactions in the EMR clearly and accurately, evaluate and prioritize symptoms based on urgency, and communicate with supportive care staff to ensure access to appropriate services. Participate in interdisciplinary team meetings to contribute to shared care planning and quality improvement.
