Job Summary
Assess consumers with co-morbidities to develop personalized care plans and provide comprehensive nursing assessments for physical and behavioral health needs. Lead interdisciplinary team meetings to review clinical data, update plans, and determine follow-up frequency while advocating for families to access community resources for chronic disease management. Utilize motivational interviewing skills to empower patients toward independence and coordinate integrated care for those with serious mental illness and addiction disorders. Maintain documentation compliance, participate in required ODMHSAS training, and possess a current RN or LPN license with CPR, First Aid, and Therapeutic Options certifications. Requires valid Oklahoma driver's license, local travel, and proficiency in MS Word.
Required Qualifications
- Current and active license to practice as a Registered Nurse or Licensed Practical Nurse
- Participation in all required ODMHSAS training related to their discipline, including understanding scope of practice as it relates to behavioral health within a certified community behavioral health agency
- Minimum of one (1) year experience in a mental health related field or experience working in a healthcare setting
- High degree of flexibility and efficiency
- Proficiency in MS Word processing software and intermediate general computer skills
- Good interpersonal skills and the ability to effectively communicate verbally with English-speaking patients
- Excellent written and oral communication skills
- Ability to move about facilities freely
- Casual professional dress with adherence to dress code requirements
- Access to reliable transportation
- Valid Oklahoma driver's license
- Current automobile liability insurance including uninsured motorist coverage
- CPR, First Aid and Therapeutic Options certifications (or obtain within 30 days of employment)
- All NorthCare Training (or complete within 30 days of employment)
- TB test within 12 months prior to employment with NorthCare (or obtain within 30 days of employment)
- Adherence to any and all laws, professional standards and ethical codes that apply to his/her job responsibilities
