Nurse Care Coordinator

Conduct comprehensive clinical and psychosocial assessments to establish rapport with patients and families, integrating cultural factors into care planning. Collaborate with physicians and interdisciplinary teams to develop and communicate plans of care, while gathering data to monitor effectiveness and coordinate patient transfers. Document all interventions, manage discharge planning, and follow up on test results to ensure efficient, cost-effective treatment progression. Monitor observation patients closely to optimize resource utilization. Requires RN/BSN with Illinois licensure and 1–2 years of healthcare experience, with 25–50% time spent walking or standing and 10% travel.
