Job Summary
Nurse Care Coordinator collaborates with the interdisciplinary team and physician to develop and execute comprehensive plans of care for patients, performing assessments and screenings of clinical, psychosocial, and health issues; build rapport with patients and families to set realistic goals; integrate cultural factors into interviews; communicate care plans to patients, families, and healthcare team; document assessments and interventions in the medical record; coordinate transfers to appropriate levels of care; monitor progress and utilization of resources; proficient in electronic health records with strong interpersonal, analytical, and time-management skills; 25-50% walking/standing with 10% travel; requires RN/BSN in Illinois with 1-2 years’ healthcare experience, preferably in care coordination; preferred experience with behavioral health clients; travel and exposure to potential infectious disease or violence.
Required Qualifications
- RN/BSN with State of Illinois
- 1-2 years' experience working in a healthcare setting, preferably in care coordination
- 25%-50% of time spent walking or standing, travel 10%
- Potential exposure to infectious disease or violence
