Job Summary

Integrate knowledge, skills, and principles of teaching and learning to support healthy lifestyle choices and chronic disease prevention, education, and management services for clients, families, the general public, and health professionals. Utilize a consultative approach, accepted standards, guidelines, and current research findings to provide support to clients and healthcare professionals as appropriate. Work relatively independently to make decisions based on sound assessments in accordance with policies of Population and Public Health Services, the health region, and the College of Registered Nurses of Saskatchewan Standards of Practice. Develop relationships with specialty programs that manage chronic conditions while maintaining confidentiality and demonstrating leadership skills within the Primary Health Care Team.

Required Qualifications

  • Minimum of one (1) year nursing experience in a primary health setting
  • Experience in teaching adults, facilitation skills, and leading group activities
  • Experience in community needs assessment/development, health promotion
  • 3 yrs exp chronic disease/complex illness Edu. or chronic disease management required
  • Three (3) years nursing experience with patients/families with chronic disease
  • Degree in Nursing with courses in supervision or degree in Edu./adult learning
  • Bachelor of Science in Nursing (BScN)
  • Valid Class 5 driver's license
  • Nursing diploma is acceptable
  • Diabetes Education certificate (CDE) is acceptable
  • Basic Cardiac Life Support Certificate (BCLS)
  • Knowledge of the impact of social/economic/cultural issues on chronic disease
  • Knowledge in learning and teaching principles
  • Demonstrated ability to maintain confidentiality of information
  • Knowledge of traditional care needs of patients of First Nations, Metis ancestry
  • Demonstrated knowledge of teaching abilities & group facilitation skills
  • Knowledge in population health promotion and its relationship to health status
  • Knowledge in Chronic Disease Management and behaviour change models
  • Experience working with patients/families with diabetes and renal disease
  • Ability to work independently and with broader interdisciplinary teams
  • Knowledge in chronic disease prevention and management
  • Expert knowledge in relevant regulatory guidelines
  • Knowledge of program planning process
  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)

Desired Qualifications

  • Nephrology Certification is desired
