Job Summary
Reporting to the COPD program manager, the nurse clinician has primary clinical responsibility for the COPD program, assessing and planning self-management care, coordinating with physicians and multidisciplinary teams, providing self-management education/counselling for clients/families/caregivers, and contributing to development, monitoring, and evaluation of programs, protocols, guidelines, and clinical standards in collaboration with leadership and the team. Travel is required; duties involve chart reviews, history taking, physical assessments, lab data review, and liaison with hospital departments and community agencies to ensure continuity of care and timely access to care while advancing COPD care and outcomes.
Required Qualifications
- Bachelor of Science in Nursing (BScN)
- Basic Life Support Certificate (BLS)
- Respiratory Therapy Certificate
- Nursing diploma acceptable
- Registered/eligible with CRNS
