Job Summary
System Flow Coordinator for 24-hour patient access service responsible for patient flow and coordination to facilitate access to safe patient care. Coordinates and accountable for decisions regarding consultation, placement and movement of patients within and outside of tertiary acute care. Integral member of an acute care team with responsibilities spanning access to physicians, patients, families, nurses and support staff; serves as a management representative for problem solving and communication during off-hours.
Required Qualifications
- Bachelor of Science in Nursing (BScN) or equivalent
- Registered/eligible with College of Registered Nurses of Saskatchewan (CRNS) OR CRPNS (as applicable)
- Basic Life Support Certificate (BLS)
- 5 years of recent experience in medical/surgical/critical care or ER
- Nursing diploma acceptable
- Bachelor of Psychiatric Nursing acceptable
- Knowledge of System Flow tools such as APF, SCM and SER
- Strong communication and leadership skills
- Ability to coordinate patient movement and access to care across continuum
- Multi-site coordination experience
