Job Summary

Direct team members of senior case managers, RNs, LPNs, and home health aides in the provision of care according to state regulations and agency policy. Assist the Regional Director in managing services, ensuring compliance with state regulations, and evaluating clinical documentation for accuracy. Plan, implement, and evaluate care while coordinating client care conferences and staffing within scope of practice. Serve as a local on-site clinical resource, participate in performance improvement initiatives, and oversee fiscal affairs including contract management and equipment tracking. Maintain current contracts, ensure employee credentialing, and assist with annual evaluations and marketing activities as requested.

Required Qualifications

  • Graduate of a state approved school of professional registered nursing
  • Current, unrestricted RN license in the state(s) of practice
  • Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
  • One (1) year of supervisory and/or case management experience
  • Current CPR certification
  • Licensed driver with clean driving record and insured in accordance with state and/or Agency requirements
  • Available at all times during operating hours

Desired Qualifications

  • BSN preferred
