Job Summary
Conduct provider-level care coordination and preventative management for Texas Independence Health Plan ISNP members, including acute and chronic illness evaluation, medication review, and Advance Care Planning education. Deliver clinical care and education to patients, families, and nursing facility staff while maintaining high professionalism and adhering to Texas Board of Nursing standards. Respond timely to compliance requests, abnormal diagnostic tests, and consultant recommendations, and efficiently document all activities in Electronic Medical Record Systems. Participate in daily morning meetings, ongoing staff education, and Quality Improvement initiatives, while fulfilling rotational weekday/weekend on-call responsibilities to ensure continuity of care. Maintain active RN and APRN licensure in Texas with current Board Certification and appropriate malpractice insurance.
Required Qualifications

- Licensed as both a Registered Nurse and an Advanced Practice Registered Nurse in the state of Texas
- Maintains an up to date Board Certification in his/her specialty
- Maintaining a Texas Statutory Prescriptive Authority Agreement with attending Physicians and/or Medical Directors
- Maintaining Texas Board of Nursing ongoing educational requirements for licensure and re-certification
- Comply with all State and Federal guidelines required to practice as a licensed Advanced Practice Registered Nurse
- Maintaining an active RN and APRN license
- Maintain Professional Medical Malpractice Liability Insurance
- Maintain adequate Medical Malpractice Insurance (generally expected to be $1m/$3m)
- Maintain reasonable and appropriate communication with the facility staff
- Efficiently navigate and operate a laptop computer
- Enter data and documentation daily into designated Electronic Medical Record Systems
- Review medications, orders, and diagnostic tests
- Respond to abnormalities appropriately, within BON approved scope of practice
- Provide ongoing evaluation and treatment options of chronic illnesses
- Care for acute exacerbations of chronic illnesses as they occur
- Provide ongoing evaluation and education to patients and families on Advance Care Planning (Goals of Care and Anticipatory planning)
- Review and respond to consultant recommendations such as psychiatric, dietary, rehabilitative therapies, and pharmacy
- Maintain a high measure of professionalism in dress and actions
- Attend and participate in daily morning meetings daily when in facilities
- Respond timely to all Compliance requests
- Report to the Clinical Operations Manager (COM)
- Maintain communication with COM and facility staff regarding Plan specific to facility and/or patient/family concerns
- Provide proactive, preventative/maintenance care for the Plan Members' medical, emotional, and behavioral conditions
- Provide clinical care and education to Plan Members
- Provide clinical care and education to the family and Nursing Facility staff
- Adhere to and be accountable to all ISNP Model of Care Provider metrics and goals
- Participate in an on-call rotation to ensure continuity of care for Plan Members after hours, weekends and holidays
- Provide ongoing evaluation and treatment options of chronic illnesses
- Care for acute exacerbations of chronic illnesses as they occur
- Communicate professionally in face to face conversations with families and staff
