Job Summary
Conduct provider-level care coordination and preventative health management for Texas Independence Health Plan members within the ISNP Model of Care. Deliver clinical care, education to patients and facility staff, and manage chronic and acute conditions while adhering to Board of Nursing standards. Operate electronic medical record systems daily, review medications and diagnostic tests, and respond to compliance requests and consultant recommendations. Maintain professionalism in dress and actions, attend daily morning meetings, and participate in on-call rotations for weekday and weekend coverage. Provide ongoing evaluation, treatment options, and Advance Care Planning education. Report to the Clinical Operations Manager and assist with Quality Improvement initiatives as needed.
Required Qualifications
- BSN
- MSN
- current Board Certification
- RN
- APN/APRN
- Licensed as both a Registered Nurse and an Advanced Practice Registered Nurse in the state of Texas
- Maintains an up to date Board Certification in his/her specialty
- Maintains a Texas Statutory Prescriptive Authority Agreement with attending Physicians and/or Medical Directors
- Maintains Texas Board of Nursing ongoing educational requirements for licensure and re-certification
- Complies with all State and Federal guidelines required to practice as a licensed Advanced Practice Registered Nurse
- Maintains an active RN and APRN license
- Maintains Professional Medical Malpractice Liability Insurance
- Maintain adequate Medical Malpractice Insurance (generally expected to be $1m/$3m)
- Texas Statutory Prescriptive Authority Agreement with attending Physicians and/or Medical Directors
- Texas Board of Nursing ongoing educational requirements for licensure and re-certification
- Active RN and APRN license
