Job Summary
Manage the ISNP program staff of NPs and RNCCs in the Laredo facilities and Upper and Lower Valley Regions, ensuring daily provider-level care coordination and compliance with the ISNP Model of Care. Oversee clinical program execution, including recruitment, training, competency development, and regular agenda-driven meetings to address staff concerns. Conduct routine chart audits, review post-hospital documentation, and perform clinical rounds to verify accurate coding and quality of care. Maintain communication with facility leadership through monthly meetings and handle weekday/weekend on-call support duties. Requires MSN, current Board Certification, and 1-2 years of SNF/ISNP MOC experience. Reports to the Director of Clinical Operations.
Required Qualifications
- MSN
- current Board Certification
- RN
- APN/APRN
- licensed as both a Registered Nurse and an Advanced Practice Registered Nurse in the state of Texas
- minimum of 1-2 years of SNF/ISNP MOC experience
- maintains an up to date Board Certification in his/her specialty
- Proven record of collaborating and partnering effectively with physicians and facility leadership
- maintaining a Texas Statutory Prescriptive Authority Agreement with attending Physicians and/or Medical Directors
- maintaining Texas Board of Nursing ongoing educational requirements for licensure and re-certification
- comply with all State and Federal guidelines required to practice as a licensed Advanced Practice Registered Nurse
- maintaining an active RN and APRN license
- maintain Professional Medical Malpractice Liability Insurance
- available during customary working hours (approximately 8:00am – 5:00pm)
- consideration for weekday/weekend on-call support duties
- Maintain adequate Medical Malpractice Insurance (generally expected to be $1m/$3m)
