Job Summary

Manage the ISNP program staff of NPs and RNCCs in Laredo and Upper/Lower Valley Regions, ensuring daily provider-level care coordination and compliance with the Texas Independence Health Plan Model of Care. Oversee clinical program execution, quality metrics, and documentation accuracy through routine chart audits, post-hospital reviews, and clinical rounds. Direct staff management including recruitment, training, and competency development while maintaining relationships with internal and external clinical customers. Conduct regular agenda-driven meetings with facility leadership and perform on-call rotations for weekday, weekend, and holiday support. Deliver MOC training to clinical staff upon hire and annually thereafter. Maintain professional standards and navigate organizational software to support utilization management and expense tracking.

Required Qualifications

  • MSN
  • current Board Certification
  • RN
  • APN/APRN
  • licensed as both a Registered Nurse and an Advanced Practice Registered Nurse in the state of Texas
  • minimum of 1-2 years of SNF/ISNP MOC experience
  • maintains an up to date Board Certification in his/her specialty
  • Proven record of collaborating and partnering effectively with physicians and facility leadership
  • maintaining a Texas Statutory Prescriptive Authority Agreement with attending Physicians and/or Medical Directors
  • maintaining Texas Board of Nursing ongoing educational requirements for licensure and re-certification
  • comply with all State and Federal guidelines required to practice as a licensed Advanced Practice Registered Nurse
  • maintaining an active RN and APRN license
  • maintain Professional Medical Malpractice Liability Insurance
  • available during customary working hours (approximately 8:00am – 5:00pm)
  • consideration for weekday/weekend on-call support duties
  • Maintain adequate Medical Malpractice Insurance (generally expected to be $1m/$3m)
