Job Summary
NP Provider for Hospice & Palliative Care per diem; provides specialized palliative care consultations across nursing homes, hospitals, or community settings as part of MJHS Hospice and Palliative Care. Responsibilities include symptom management, goal-setting discussions, hospice eligibility review, independent practice within an interdisciplinary team, care planning, documentation, and education; participates in on-call rotations and may travel within the assigned geographic area to patient care locations. Requires Master’s degree in Nursing and NP program completion; NYS RN licensure and NP certification in Adult, Family, Gerontology, or Palliative Care; strong communication, teaching, and organizational skills; proficiency with Microsoft Word/Excel; ability to travel as needed; ability to work in a professional, respectful manner with diverse patients and families.
Required Qualifications
- Master's degree in Nursing
- Graduate of an approved Nurse Practitioner Program
- New York State RN licensure
- Certified Adult, Family, Gerontology, or Palliative Care Nurse Practitioner
- CPR-BLS preferred
- valid NYS Driver's license preferred
- Hospice & Palliative Care certification strongly encouraged by year 2 of hire
- Microsoft Word and Excel
