Job Summary
Implement specialist palliative care consultation services by conducting initial and follow-up visits for patients with life-limiting conditions in community or facility settings. Manage symptom control, facilitate goal-setting discussions, and review hospice eligibility while collaborating with an interdisciplinary team. Perform timely, accurate, and compliant documentation for each encounter, adhering to best practices in history-taking, examination, care planning, and treatment implementation. Support hospice operations through participation in provider on-call rotations, developing quality initiatives, and educating staff. Maintain effective communication with referral sources and demonstrate professional courtesy with patients and families across diverse backgrounds.
Required Qualifications
- Master's degree in Nursing
- Graduate of an approved Nurse Practitioner Program
- New York State RN licensure
- Certified Adult, Family, Gerontology, or Palliative Care Nurse Practitioner
Desired Qualifications
- Minimum of 1-year clinical experience as a NP provider
- Hospice, Palliative Care and/or Geriatric nursing or NP experience
- CPR-BLS
- valid NYS Driver's license
- Hospice & Palliative Care certification
- Microsoft Word and Excel
- Ability to work flexible hours to meet the needs of the position
- Ability to coordinate the resources and personnel involved in a comprehensive interdisciplinary team environment
- Ability to travel within the assigned geographic area to provide patient care
- Possess teaching, communication, and listening skills
- Ability to work non-judgmentally with patient / family members of any culture, religion, socio-economic background or lifestyle
- A high degree of accountability
- Excellent communication, listening, and organizational skills
- Ability to work within an integrated health care team
- Ability to demonstrate effective critical thinking skills
