NP Provider Hospice & Palliative Care (Pediatric)

Job Summary
Pediatric NP provider in hospice/palliative care delivering in-person visits across Manhattan, Bronx, Brooklyn, Queens, and Long Island with telehealth as appropriate. Responsibilities include comprehensive NP evaluations for hospice eligibility, care planning and coordination with interdisciplinary teams, in-home/community and occasional facility-based consultations, documentation, and contribution to teaching curricula on pediatric hospice care. Requires direct patient care and collaboration with a physician and manager, plus training role within the hospice team and participation in quality improvement activities. Must hold NY NP license (Pediatric/Family/Palliative), CPR-BLS, valid NY driver’s license with access to a car, and be prepared for flexible hours and travel within the geographic area. Prior pediatric NP or hospice experience strongly preferred; certification in Hospice & Palliative Care encouraged by year 2. Proficiency with Microsoft Word/Excel and strong communication/listening skills are essential.
Required Qualifications
- Master’s degree in nursing from an approved Nurse Practitioner program
- NY State NP License (Certified Pediatric, Family, or Palliative Care NP)
- CPR-BLS
- NY State Driver’s license and access to car
- Hospice & Palliative Care certification strongly encouraged by year 2 of hire
- Microsoft Word and Excel
- Ability to work flexible hours and coordinate interdisciplinary team
- Experience in hospice/palliative care settings or pediatric care preferred (1 year clinical NP experience preferred)
- Ability to travel across geographic territory to provide patient care
- Excellent communication, listening, and organizational skills
- Teaching and training capability for hospice team
- Evidence-based approach to management and patient care
- Quality improvement participation
