Job Summary
Pediatric NP Provider Hospice/Palliative Care position covering field-based settings in Manhattan, Bronx, Brooklyn, Queens, and Long Island. Responsibilities include providing NP-appropriate hospice/palliative care to pediatric patients enrolled in hospice, offering in-person or telehealth visits, coordinating with the interdisciplinary team, evaluating for hospice eligibility, contributing to teaching curricula on pediatric hospice care, and supporting quality improvement initiatives. Requires an approved NP program, NYS NP license (pediatric, family, or palliative), CPR-BLS, and ability to travel across the geographic territory. Strong emphasis on communication, organization, teaching, and collaborative patient-family-centered care within a hospice and community-based care environment.
Required Qualifications
- Master’s degree in nursing and graduate of an approved Nurse Practitioner Program
- NYS NP License (Certified Pediatric, Family, or Palliative Care NP)
- CPR-BLS
- NYSDRIVER'S LICENSE AND CAR
