Job Summary
Pediatric NP Provider Hospice/Palliative Care responsible for providing NP-appropriate services to patients enrolled in hospice and may assist in staffing pediatric palliative care consultations. Works in community settings and may provide occasional hospital or long-term care consultations, with in-person visits or telehealth as appropriate. Responsibilities include history-taking, physical examinations, care planning, treatment recommendations, documentation, coordinating with the interdisciplinary team, contributing to teaching and curriculum development in pediatric hospice care, and supporting quality-improvement initiatives. May travel across the geographic territory to provide patient care and support for colleagues, with opportunities to teach and train hospice staff and contribute to program development.
Required Qualifications
- Master’s degree in nursing from an approved Nurse Practitioner program
- NY state NP license (Certified Pediatric, Family, or Palliative Care NP)
- CPR-BLS
- NY state driver’s license and car
- Hospice & Palliative Care certification strongly encouraged by year 2 of hire
