NP Provider Hospice & Palliative Care (Pediatric)

Job Summary
Provide NP-based hospice and pediatric palliative care in community or facility-based settings. Deliver in-person visits or telehealth as appropriate, coordinating with the interdisciplinary team to assess, plan, and implement hospice eligibility and palliative care plans. Contribute to training of hospice staff, develop pediatric aspects of hospice care curricula, and engage in quality improvement activities. Requires Master’s in nursing and completion of an approved NP program; NYS NP license; CPR-BLS; willingness to travel; strong communication, teaching, and interdisciplinary collaboration skills.
Required Qualifications
- Master’s degree in nursing
- Graduate of an approved Nurse Practitioner Program
- NYS NP License (Certified Pediatric, Family, or Palliative Care NP)
- CPR-BLS
- NYSD driver’s license and car
- Microsoft Word and Excel
- Ability to work flexible hours and coordinate interdisciplinary teams
- Ability to travel across geographic territory to provide patient care
- Pediatric nursing or pediatric NP experience strongly preferred
- Hospice & Palliative Care certification encouraged by year 2 of hire
