Job Summary
The Pediatric NP Provider Hospice/Palliative Care role involves delivering NP-appropriate hospice/palliative services in community and occasional facility/hospital settings, including in-person visits or telehealth, collaborating with the interdisciplinary team, and contributing to teaching curriculum and quality improvement initiatives. Responsibilities include face-to-face evaluations for hospice eligibility, management of hospice patients, and providing both direct patient care and support to clinicians with an evidence-based approach, plus potential cross-coverage for pediatric care within hospice and palliative programs.
Required Qualifications
- Master’s degree in nursing and graduate of an approved Nurse Practitioner Program
- NYS NP License (Certified Pediatric, Family, or Palliative Care Nurse Practitioner)
- CPR-BLS
- NYS Driver's license and car
- Hospice & Palliative Care certification strongly encouraged by year 2 of hire
