Job Summary
Provide NP-appropriate service to patients enrolled in hospice and assist in staffing pediatric palliative care consultation services. Conduct in-person or telehealth visits to deliver comfort care consistent with best practices, including history-taking, examination, care planning, and documentation. Coordinate with the team physician and manager for face-to-face evaluations regarding hospice eligibility and exigent medical care needs. Contribute to interdisciplinary team training and help build a teaching curriculum on pediatric aspects of hospice care through cross coverage. Utilize an evidence-based approach to management while participating in quality improvement projects as appropriate.
Required Qualifications
- Master's degree in nursing
- graduate of an approved Nurse Practitioner Program
- NYS NP License (Certified Pediatric, Family, or Palliative Care Nurse Practitioner)
- CPR-BLS
- NYS Driver's license

Desired Qualifications
- Minimum of 1-year clinical experience as a NP provider
- Prior experience working as a member of a specialist-level palliative care team
- Prior experience working in a hospice environment
- Prior pediatric nursing experience
- Prior pediatric NP experience
- Hospice & Palliative Care certification
