NP Provider Hospice & Palliative Care (Pediatric)

Job Summary
Provide NP-appropriate service to patients enrolled in hospice and pediatric palliative care, conducting in-person or telehealth visits for comfort care, history-taking, examination, and care planning. Coordinate with team physicians to manage exigent medical care, perform face-to-face evaluations for hospice eligibility, and contribute to interdisciplinary team training and curriculum development on pediatric aspects of hospice care. Utilize an evidence-based approach to treatment recommendations and documentation while supporting quality improvement projects. Travel across the agency's geographic territory in Manhattan, Bronx, Brooklyn, Queens, and Long Island to serve patients and families in community, long-term care, or hospital settings.
Required Qualifications
- Master's degree in nursing
- graduate of an approved Nurse Practitioner Program
- NYS NP License (Certified Pediatric, Family, or Palliative Care Nurse Practitioner)
- CPR-BLS
- Driver's license and car
- Ability to work flexible hours to meet the needs of the position
- Ability to travel across the agency's geographic territory to provide patient care as needed
- Possess teaching, communication, and listening skills
- Ability to work non-judgmentally with patient / family members of any culture, religion, socio-economic background, or lifestyle
- A high degree of accountability
- Excellent communication, listening, and organizational skills
- Ability to work within an integrated health care team
- Ability to demonstrate effective critical thinking skills
- Ability to problem solve independently
Desired Qualifications
- Minimum of 1-year clinical experience as a NP provider
- Prior experience working as a member of a specialist-level palliative care team or working in a hospice environment
- Prior pediatric nursing or pediatric NP experience
- Hospice & Palliative Care certification
