Job Summary

Provide NP-appropriate service to patients enrolled in hospice and assist in staffing pediatric palliative care consultation services. Conduct in-person or telehealth visits to deliver comfort care consistent with best practices, including history-taking, examination, care planning, and documentation. Coordinate with the team physician and manager for face-to-face evaluations of hospice eligibility and management of patients requiring exigent medical care. Contribute to team training and help build a teaching curriculum on pediatric aspects of hospice care through cross coverage. Perform quality improvement projects as appropriate while using an evidence-based approach to management and policy adherence. Travel across the agency's geographic territory to provide patient care in community, long-term care, or hospital settings.

Required Qualifications

  • Master's degree in nursing
  • Graduate of an approved Nurse Practitioner Program
  • NYS NP License (Certified Pediatric, Family, or Palliative Care Nurse Practitioner)
  • CPR-BLS
  • NYS Driver's license
  • Car

Desired Qualifications

  • Minimum of 1-year clinical experience as a NP provider
  • Prior experience working as a member of a specialist-level palliative care team
  • Prior experience working in a hospice environment
  • Prior pediatric nursing or pediatric NP experience
  • Hospice & Palliative Care certification
  • Ability to work flexible hours to meet the needs of the position
  • Ability to coordinate the resources and personnel involved in a comprehensive interdisciplinary team environment
  • Ability to travel across the agency's geographic territory to provide patient care as needed
  • Possess teaching, communication, and listening skills
  • Ability to work non-judgmentally with patient / family members of any culture, religion, socio-economic background, or lifestyle
  • A high degree of accountability
  • Excellent communication, listening, and organizational skills
  • Ability to work within an integrated health care team
  • Ability to demonstrate effective critical thinking skills
  • Ability to problem solve independently
  • Microsoft Word and Excel
