Job Summary
Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients. Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations. Monitor patient progress closely and adjust treatment plans as necessary. Maintain accurate and timely documentation of initial evaluations, progress notes, and discharge summaries in electronic medical records systems. Manage a caseload of 15-20 patients daily in a 100% inpatient setting with specialists support for IM, Nephrology, Pulmonology, Wound Care, Psychiatry, PM&R, and Pain. No in-house call required. Flexible hours available including 7 on 7 off, M-F, or weekend schedules.
Required Qualifications
- Physician Assistant (PA) or Nurse Practitioner (NP) degree
- Valid medical license in the state of practice
- Strong clinical skills in the evaluation and management of patients with acute rehabilitation needs
- Excellent interpersonal and communication skills with the ability to collaborate effectively within a multidisciplinary team
- Maintain accurate and timely documentation of patient evaluations and progress notes
- Demonstrated leadership abilities

- 15-20 patients daily
- Flexible hours 7 on 7 off; M-F or Weekend schedules available
