Job Summary
NP/PA – Geriatric Home Based Primary Care APP delivering care to geriatric, frail, and medically complex patients across diverse living situations. Works within a collaborative, multidisciplinary team but maintains independence with autonomous clinical practice. Full-time position (five 8-hour days, 40 hours/week) located in multiple West Michigan communities (Byron Center, Big Rapids, Rockford, Greenville, Fremont, Lakeview, Reed City).
