Job Summary
Provide comprehensive care to seniors following clinical standards while collaborating daily with a multidisciplinary team to ensure coordinated, high-quality outcomes. Manage referrals, hospital/SNF coordination, DME, and home health services, and document care accurately with support from quality-based coders. Participate in on-call rotation and support growth plans at the center. Work onsite at the Seminary clinic in Fort Worth, TX, reporting to the Associate Medical Director for Dallas, TX. Requires bilingual (English/Spanish) credentials, a Master's degree or PA program completion, and an active, unrestricted license.
Required Qualifications
- Bilingual (English/Spanish)
- Master's Degree in Nursing or completion of a PA program with board certification
- Active, unrestricted NP or PA license in the state of practice
Desired Qualifications
- Experience in value-based care, primary care or Medicare Advantage settings
- Active DEA license and Medicare/Medicaid provider numbers
- Ability to obtain DEA license and/or Medicaid and Medicare numbers
- Familiarity with HEDIS quality indicators and Medicare guidelines
