Job Summary
HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) conducting annual in-home health assessments for Medicare Advantage and other plan members across the state of Washington. Non-prescribing, field-based role focused on education, gap closure, and collaboration with primary care providers to improve health outcomes. Travel approximately 85% within WA; base salary plus incentives; live anywhere in the state. Flexible scheduling (8am–8pm over a 7-day week) with no on-call, weekends, or holidays. Responsibilities include comprehensive in-home assessments (medical history, medications, vitals, physical exam), screenings and point-of-care testing, documenting diagnoses for care management, communicating findings to PCPs, recognizing urgent situations, educating members about diseases and medications, addressing social determinants of health, and providing referrals. Requires NP or PA certification and active state licensure, a valid driver’s license, and ability to travel to diverse home environments. Preferred qualifications include 1+ years of clinical experience and proficiency with EMRs. Compensation ranges from $109,500–$164,000 plus benefits and incentives.
Required Qualifications
- Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care (NP)
- Current NCCPA certification and state licensure (PA)
- Active, unrestricted licensure in the practicing state
- Active and unrestricted driver's license and access to reliable transportation for home visits
- Ability to obtain or maintain licensure where required by state law
- Ability to conduct in-home assessments and navigate varied home environments
- No on-call, weekends, or holidays requirement (as described)
