Job Summary
Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) for Optum HouseCalls traveling statewide in WA. Conduct in-home health assessments for Medicare Advantage and other plan members; non-prescribing, field-based role focused on education, gap closure, and care coordination with primary care providers. Approximately 85% travel across the state; live anywhere in WA. Flexible scheduling (8am–8pm within a 7-day week) with no on-call, weekends, or holidays. Responsibilities include comprehensive in-home assessments (PMH, medications, vitals, exam), screenings and point-of-care testing, documenting diagnoses for care management, communicating findings to PCPs, identifying urgent situations, patient education on diseases and medications, and addressing social determinants of health with referrals as needed. Preferred qualifications include 1+ years of clinical experience in family/geriatric/home health, EMR proficiency, ability to transport equipment up to 30 lbs, and strong communication with geriatric/Medicare populations. Compensation ranges from $109,500–$164,000 with benefits and incentive programs; equal opportunity employment.
Required Qualifications
- Active ANCC or AANP certification for NP or
- Current NCCPA certification for PA
- State licensure (active, unrestricted)
