Job Summary
Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) conducts in-home health assessments for Medicare Advantage members across Washington with ~85% travel; non-prescribing, field-based role focusing on education, gap closure, and collaboration with primary care providers. Responsibilities include comprehensive in-home assessments (history, meds, vital signs, exams), evidence-based screenings, documenting diagnoses for care management, communicating findings to PCPs to address care gaps, recognizing urgent conditions, educating members about diseases and medications, and addressing social determinants of health with referrals. Requires active NP/PA certifications and state licensure; preferred 1+ years of clinical experience; proficient with electronic medical records; must be able to transport equipment up to 30 pounds and navigate stairs. Flexible scheduling is offered; no on-call, weekends, or holidays in scope. Compensation ranges from $109,500 to $164,000 plus benefits and incentives. Required Qualifications
- Active ANCC or AANP certification for NPs OR NCCPA certification and state licensure for PAs
- Active, unrestricted state licensure without disciplinary actions
- Active and unrestricted driver's license and reliable transportation
- Ability to navigate varied home environments and perform assessments
- If applicable, approval from licensing board for independent practice (within specified timelines)
