Job Summary
HouseCalls Advanced Practice Clinician (NP or PA) conducting comprehensive in-home health assessments across Washington state for Medicare Advantage and other plans. Role emphasizes education, gap closure, care coordination with PCPs, and population health outcomes in an interdisciplinary team. Travel is extensive (approximately 85%), with all travel expenses covered; base salary plus incentives. Flexible scheduling (8am–8pm, 7 days) with no on-call, weekends, or holidays. Requires licensure in WA (and eligibility for independent practice where applicable) and travel-friendly capabilities, including ability to navigate varied home environments and transport equipment. Primary responsibilities include medical history review, physical exams, vital signs, medication reconciliation, point-of-care testing as appropriate, documenting diagnoses for care planning, communicating findings to PCPs, recognizing urgent situations, patient education on diseases and medications, and addressing social determinants of health with referrals as needed. Preferred qualifications include 1+ year of clinical experience in family, geriatric, or home health care, proficiency with EMR/EHR, ability to transport up to 30 pounds, and strong communication with geriatric/Medicare populations. Required Qualifications
- Active ANCC or AANP certification for NP or
