Job Summary
HouseCalls NP or PA conducting in-home health assessments across the state of Washington (travel ~85%), non-prescribing field-based role focused on education, gap closure, and coordination with PCPs to improve health outcomes. Responsibilities include comprehensive in-home assessments (history, meds reconciliation, vitals, exam), screenings and point-of-care testing, documenting diagnoses for care management, communicating findings to PCPs, recognizing urgent situations, patient education on diseases and medications, and addressing social determinants of health with referrals as needed. Requires active NP or PA certification, state licensure, and a valid driver’s license with reliable transportation; travel across WA with a base salary plus incentives; flexible scheduling (8am-8pm, 7-day week) with no on-call, weekends, or holidays. Emphasis on collaboration with interdisciplinary team, remote/field-based care delivery, and improving access to care for Medicare Advantage and other plan members.
Required Qualifications
- Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care for NPs OR Current NCCPA certification and state licensure for PAs
- Active, unrestricted licensure in the practicing state (or ability to obtain by start date) without current disciplinary actions or practice limitations
- Active and unrestricted driver’s license and reliable transportation for home visits
- Ability to navigate varied home environments and perform assessments (standing, sitting, kneeling)
- If applicable by state, approval from licensing board to practice independently (if required by state) and timely application for eligibility
