NP or PA, HouseCalls Statewide Traveler - WA

Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) conducts in-home health assessments for Medicare Advantage and other plan members in Washington state. This field-based role requires travel across the state (approximately 85%), with all travel expenses covered. No prescribing is involved; focus is on education, gap closure, and collaboration with PCPs to improve health outcomes. Responsibilities include comprehensive in-home assessments, medication reconciliation, vitals, point-of-care testing as appropriate, documenting diagnoses for care management, communicating findings to PCPs, identifying urgent situations, educating members on diseases and medications, and addressing social determinants of health with referrals as needed. Preferred qualifications include 1+ years of clinical experience in family, geriatric, or home health, proficiency with electronic medical records, ability to transport equipment up to 30 pounds, and strong communication with geriatric/Medicare populations. Compensation ranges from $109,500 to $164,000 plus benefits and incentive programs. Requires active certification (ANCC/AANP for NPs or NCCPA for PAs) and state licensure, with valid driver’s license and reliable transportation for home visits. No on-call, weekends, or holidays; flexible scheduling within a 8am-8pm window across a 7-day week.
