HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) conducts in-home health assessments across Washington state (approx. 85% travel). Role is field-based, non-prescribing, focusing on education, gap closure, and collaboration with PCPs to improve health outcomes. Responsibilities include comprehensive in-home assessments with medical history review, vital signs, medication reconciliation, point-of-care testing as appropriate, documenting diagnoses for care management, communicating findings to PCPs, recognizing urgent situations, educating patients on diseases and medications, and addressing social determinants of health with referrals as needed. Preferred qualifications include 1+ years of clinical experience in family, geriatric, or home health, proficiency with EMRs, ability to transport equipment up to 30 pounds, and strong communication with Medicare populations. Compensation ranges from $109,500 to $164,000 plus comprehensive benefits, incentives, and 401k. No weekends or holidays and flexible scheduling offered. All candidates must maintain active licensure and certification as applicable, and undergo pre-employment drug testing. Locations listed indicate West Seattle, WA; travel throughout the state is required.
