Job Summary
Provide daily rounding on patients in skilled nursing facilities (SNF) and long term care (LTC) settings as the primary NP or PA for one to two SNFs, managing patients across skilled and long term care levels. Complete an average of 6-9 patient encounters per day (8-13 total daily census across facilities). Participate in weekly interdisciplinary team meetings and collaborate with Extended Care physicians and primary care teams to align care with patient goals and deliver high quality, value based care. This role supports OptumCare's mission within the Puget Sound region and emphasizes patient-centered care, teamwork, and ongoing professional development.
Required Qualifications
- Current Washington state license, or ability to obtain prior to hire
- Nurse Practitioner Certification (NP-C) or Physician Assistant (PA-C) certification, or ability to obtain before hire
- Strong communication skills
