Job Summary
Geriatric NP or PA to join OptumCare in Mukilteo, WA, providing daily rounding in skilled nursing facilities and long-term care. Serve as primary provider for one to two SNFs, managing patients across skilled and long-term care levels. Complete 6-9 patient encounters per day with daily census ~8-13 patients. Participate in weekly interdisciplinary team meetings and collaborate with Extended Care physicians and the patient’s primary care team to align care with patient goals in value-based care. Requires WA state license and NP or PA certification. Preferred: experience in SNF/long-term care, geriatrics, complex care management, palliative care. Compensation range $109,500-$164,000 plus comprehensive benefits; equal employment opportunity employer; drug test required.
Required Qualifications
- Current Washington state license, or ability to obtain prior to hire
- Nurse Practitioner Certification (NP C) or Physician Assistant (PA C) certification, or ability to obtain before hire
- Strong communication skills, a record of high patient satisfaction, and demonstrated ability to promote effective teamwork
