Job Summary

Evaluate, diagnose, and manage patients with advanced heart failure in the Heart Failure Bridge Clinic, developing individualized treatment plans and optimizing guideline-directed medical therapy. Assess volume status, initiate IV diuretic therapy, and coordinate transitions between inpatient and outpatient care to reduce hospital readmissions. Collaborate with physicians, pharmacists, and multidisciplinary team members to deliver comprehensive care, while educating patients and families on management, medications, and lifestyle modifications. Document and bill for professional services, supporting evidence-based practice and quality improvement initiatives. Work a four 10-hour weekday schedule with no weekends, holidays, or call responsibilities.

Required Qualifications

  • NP: Master's or Doctorate in Nursing with Acute Care Nurse Practitioner or Family Practitioner certification
  • Current Maryland NP (or eligibility)
  • Minimum of two (2) years of recent cardiology experience as a practicing NP
  • Experience managing patients with cardiovascular disease in an in-patient and/or outpatient setting
  • Strong clinical judgment, critical thinking, and decision-making skills
  • Excellent communication and interpersonal skills with a commitment to collaborative, patient-centered care
