Job Summary
Independently evaluate, diagnose, and manage patients with advanced heart failure in the Heart Failure Bridge Clinic. Develop individualized treatment plans, optimize guideline-directed medical therapy, and assess volume status while initiating IV diuretic therapy. Collaborate with Advanced Heart Failure physicians and a multidisciplinary team to coordinate transitions between inpatient and outpatient care, educate patients on management strategies, and support quality improvement initiatives. Document and bill for professional services within a specialized outpatient setting featuring four 10-hour weekday shifts with no weekends, holidays, or call responsibilities.
Required Qualifications
- NP: Master's or Doctorate in Nursing with Acute Care Nurse Practitioner or Family Practitioner certification
- Current Maryland NP (or eligibility)
- Minimum of two (2) years of recent cardiology experience as a practicing NP
- Experience managing patients with cardiovascular disease in an in-patient and/or outpatient setting
- Strong clinical judgment, critical thinking, and decision-making skills
- Excellent communication and interpersonal skills with a commitment to collaborative, patient-centered care
