Job Summary
Independently evaluate, diagnose, and manage patients with advanced heart failure in the outpatient Bridge Clinic by developing individualized treatment plans, optimizing guideline-directed medical therapy, and initiating IV diuretic interventions. Collaborate with Advanced Heart Failure physicians and a multidisciplinary team to coordinate inpatient-to-outpatient transitions, educate patients on symptom monitoring and lifestyle modifications, and support quality improvement initiatives. Document and bill for professional services while maintaining continuity of care. This role features a four 10-hour weekday schedule with no weekends, holidays, or call responsibilities.
Required Qualifications
- Master's or Doctorate in Nursing with Acute Care Nurse Practitioner or Family Practitioner certification
- Current Maryland NP (or eligibility)
- Minimum of two (2) years of recent cardiology experience as a practicing NP
- Experience managing patients with cardiovascular disease in an in-patient and/or outpatient setting
- Strong clinical judgment, critical thinking, and decision-making skills
- Excellent communication and interpersonal skills with a commitment to collaborative, patient-centered care
