Job Summary
Independently evaluate, diagnose, and manage patients with advanced heart failure in the Heart Failure Bridge Clinic by developing individualized treatment plans and optimizing guideline-directed medical therapy. Assess volume status to initiate IV diuretic therapy and coordinate transitions between inpatient and outpatient care to reduce hospital readmissions. Educate patients and families on management, medications, lifestyle modifications, and symptom monitoring while collaborating with physicians, nurses, pharmacists, and case managers. Document and bill for professional services while supporting quality improvement initiatives and evidence-based practice. Maintain four 10-hour weekday schedules with no weekends, holidays, or call responsibilities.
Required Qualifications
- NP: Master's or Doctorate in Nursing with Acute Care Nurse Practitioner or Family Practitioner certification
- Current Maryland NP (or eligibility)
- Minimum of two (2) years of recent cardiology experience as a practicing NP
- Experience managing patients with cardiovascular disease in an in-patient and/or outpatient setting
- Strong clinical judgment, critical thinking, and decision-making skills
- Excellent communication and interpersonal skills with a commitment to collaborative, patient-centered care
