Job Summary
Provide in-home dialysis support as a Care Partner for patients undergoing dialysis at home. Responsibilities include setting up dialysis treatments with Tablo/NxStage or equivalent machines, providing companionship, monitoring safety, assisting with light ADLs, maintaining communication with clinic nurses and care coordinators, and documenting any concerns observed during support hours. Emphasis on compassionate, reliable care, adherence to treatment plans, and collaboration with a clinical team to ensure patient wellbeing.
Required Qualifications
- Prior experience supporting dialysis patients (home or clinical setting)
- Familiarity with home dialysis machines (preferred)
- Reliable, empathetic, and detail-oriented
- Strong interpersonal communication
- Must be dependable
