Job Summary
Assess, diagnose, plan, implement, and evaluate treatment and care for service users with complex physical, mental, and substance use challenges. Prescribe and review medication for therapeutic effectiveness, prioritizing health problems and intervening in urgent situations with immediate advice from the Clinical Lead when needed. Assess, update, and refer service users for BBV vaccination and immunisation programmes while supporting them to adopt healthy lifestyle strategies. Communicate effectively with service users and carers, maintaining accurate records of consultations and participating in self and peer review to assess care delivery effectiveness.
Required Qualifications
- Qualified pharmacist with current GPhC registration
- Two years' post qualification experience
- First level registered nurse with a current PIN
- Three years post qualification experience
- Qualified Non-Medical Prescriber
- At least one year's relevant and recent experience of working within substance misuse treatment
- Sound and demonstrable knowledge of harm reduction and health promotion interventions
- Ability to assess and recovery plan service users
- Ability to formulate written reports
- Ability to communicate confidently and effectively, verbally and in writing
- Full driving licence
- Access to own transport
- Ability to demonstrate easy movement between services
- Enhanced level Disclosure and Barring Service (DBS) check
