Job Summary
Non-Clinical Case Manager (RN) responsible for coordinating comprehensive non-clinical care and resources for service members within VTARNG, maintaining HIPAA-compliant records, and coordinating treatment plans with medical facilities. Responsibilities include establishing and maintaining electronic case profiles, tracking care from identification to resolution, preparing records for MEB/PEB, and generating monthly status reports across systems (eCase, MEDCHART, MEDPROS, eProfile, MHA/PHA, AHLTA, AVS, MODS, HRR). Requires coordinating with multiple organizations, strong communication, and adherence to privacy guidelines. Based at Green Mountain Armory, Colchester, VT, with standard hours 0730–1630 and occasional weekend events as required.
Required Qualifications
- Registered Nurse (RN) with an active, unrestricted Vermont state license
- Experience in non-clinical case management or care coordination
- Ability to coordinate services across multiple organizations
- Strong organizational, communication, and interpersonal skills
- Ability to work effectively with military service members and Government personnel
- Proficiency in written and spoken English
- Ability to obtain a Common Access Card (CAC)
- Maintain HIPAA and PII compliance at all times
