Job Summary
Provide hands-on nursing care in accordance with scope of practice, administer medications and treatments, and monitor for effectiveness. Collaborate with RNs and physicians during rounds and care planning, coordinate care, and communicate changes to residents' families and team members. Document all care accurately and timely, complete admission documentation, and assist in discharge planning. Ensure safe, clean, and well-stocked medication and treatment carts while supervising Medication Aides/Techs during medication administration. Support audits and assist with quality improvement efforts. Requires current LPN license, CPR certification, and six months experience in a long-term care environment. Schedule includes Tuesday, Wednesday, and Thursday 3 12s.
Required Qualifications
- Current LPN license (in good standing)
- Strong clinical knowledge and attention to detail
- Excellent communication skills with residents, families, and staff
- Calm under pressure and able to act decisively in emergencies
- A team-focused, resident-first mindset
- Dependability and punctuality for scheduled shifts
- Graduate of an accredited RN School of Nursing or LPN Program
- Current RN or LPN Licensure in the State employed
- Valid CPR Certification
Desired Qualifications
- Six months experience in a long-term care environment preferred
