Job Summary
Care Navigator role delivering remote monitoring and therapeutic exercise to support musculoskeletal health. Responsibilities include guiding patients through home-exercise therapy, providing neuromuscular re-education and functional training via our platform, supplying progress feedback to clinicians, maintaining records, improving platform processes, and managing a caseload. Requires NY PT licensure and active license in a compact state; remote-work environment and startup culture. Strong patient focus, multitasking ability, and cross-functional collaboration are essential.
Required Qualifications
- Graduated from an accredited physical therapy program
- Active PT license in New York
- Active PT license in a compact state
- Tech Savvy and comfortable with multiple platforms (EHR, scheduling, messaging)
- Excellent communication and interpersonal skills
- Ability to work independently with limited supervision
- Experience with musculoskeletal health (2+ years a plus)
