Job Summary
Conduct home visits to clients in designated geographic territories in Lebanon, MO, performing thorough evaluations of functional status, muscle function, and ADL performance. Assess cognitive, psychological, and motor aspects to develop individualized intervention plans and evaluate home environments for safety hazards. Identify equipment needs and environmental adaptations, then continually revise care plans while collaborating with nursing on multidisciplinary goals. Educate patients and family members on rehabilitative care and activities to promote independent living, and accurately document all observations and interventions using a touch pad tablet.
Required Qualifications
- A current Missouri Occupational Therapist license
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
- Ability to work independently
- Ability to manage time effectively
- Strong interpersonal skills
- Organizational skills
- Problem-solving skills
- Solid computer skills
- Ability to travel locally to cases as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
