New Grad LPN

Provide safe and respectful nursing care to an assigned group of patients or residents according to policies and individualized care plans. Conduct head-to-toe assessments, monitor for abnormal signs or symptoms, and communicate with the RN/RPN as needed. Communicate with physicians regarding patient care, transcribe and process orders, and document care provided. Prepare patients for surgical or diagnostic procedures, administer medications, and monitor their effects. Identify and respond to patient, resident, and family learning needs, providing educational materials and incorporating teaching activities into care plans. Respond to incidents such as Codes, injuries, hazardous spills, aggressive behavior, and fire alarms, and fill out appropriate forms. Participate in the development, modification, and evaluation of individualized care plans with the interdisciplinary team. Assist with discharge planning by considering patient or resident status, external supports, and other relevant factors.
