Job Summary
Provide specialized outpatient nephrology care as a Nurse Practitioner in a multi-specialty Bronx practice. Conduct comprehensive patient assessments, manage kidney-related conditions (CKD, hypertension, electrolyte imbalances, glomerulonephritis, ESRD), order/interpret diagnostic tests (creatinine, BUN, GFR, urinalysis, imaging), develop and implement individualized treatment plans in collaboration with supervising nephrologist, prescribe medications (antihypertensives, diuretics, phosphate binders), educate patients and families on kidney disease management and lifestyle modifications, coordinate care with dialysis centers and transplant teams, monitor progress with regular follow-ups, maintain accurate EHR documentation, and stay current with nephrology advancements through CME and collaboration with physicians.
Required Qualifications
- Master's degree in Nursing
- Active New York State license
- Board-certified or board-eligible
- At least 1 year of experience as a Nurse Practitioner
